Do I accept insurance?
Yes, I am in-network with Premera Blue Cross, LifeWise, some BCBS plans, First Choice Health, Kaiser PPO, and Lyra Health.
If I am not paneled with your insurance provider, I can provide you a superbill for you to submit to insurance for reimbursement.
What are accepted methods of payment?
I accept Cash, Check, Credit, HSA/FSA cards.
What is the cancellation policy?
My cancellation policy requires at least 48 hours before your scheduled appointment to avoid a late cancellation/no-show fee of $100. I understand that unforeseen circumstances occur and so I waive one no-show or late cancellation fee per calendar year.
